Admission Criteria

Anyone can be admitted to our Hospice program, regardless of diagnosis, if they meet the following criteria:

be a resident of Cortland County

have a life expectancy of less than six months - as determined by their physician

have someone responsible and available to meet the patient's basic needs in the home
